What is shockwave therapy (ESWT) and how does it work?

The Orthospec™ utilizes electrohydraulic, spark gap technology. The treatment is a non-invasive, outpatient procedure intended to treat chronic heel pain caused by Plantar Fasciitis. The treatment regimen calls for one 25-minute session providing a total of 3,800 shocks, performed by a qualified medical professional. No anesthesia or sedation is provided, and imaging is not required.

Should I stop taking NSAIDs before shockwave treatment?

Patients do not have to discontinue NSAIDs prior to the procedure — they can stay on them pre- and post-treatment. However, it is preferred to wean off NSAIDs 2 weeks after treatment. Patients taking medications that may prolong bleeding (such as aspirin) should consult their physician regarding temporary discontinuation.

How soon can I resume normal activity after treatment?

Normal activity can be resumed immediately after shockwave treatment.

How soon can I resume strenuous activity such as running?

Strenuous activity should be delayed for approximately 2 weeks post-treatment, especially if you experience an increase in pain from your baseline level.

Should I apply heat or ice after shockwave treatment?

Heat or ice does not need to be applied post-treatment; however, your doctor can use their discretion for the application of either based on your individual comfort.

Will plantar fasciitis come back after treatment?

The Orthospec™ provides symptomatic relief from chronic pain caused by plantar fasciitis by stimulating the tissue to heal itself. While results can last from 3–6 months or longer, the fascia can become inflamed again over time. 86% of patients maintained reduced pain levels out to 3 months post-treatment in clinical studies.

How many treatments might be needed?

Based on overseas studies and current usage, physicians typically do not exceed more than 4 treatments (4 separate visits). The treating physician uses their own discretion based on the severity of the patient's condition.

How long between treatments?

A period of roughly 3 weeks between treatments is recommended to allow the treated area time to heal. Multiple treatments are not recommended in a single visit.

What is the anticoagulant protocol for patients on Coumadin?

For patients on anticoagulant therapy such as Coumadin, it is suggested they come off the therapy 7 days prior to treatment and resume immediately post-treatment. An INR assessment is suggested to gauge blood coagulation.

Can ESWT be used alongside other therapies like orthotics or physical therapy?

Yes. Orthotic use is recommended post-treatment to decrease the risk of recurrence. Patients should also continue a stretching program to allow for elongation of the plantar fascia. Steroid injections should have been exhausted prior to ESWT. If pain increases, palliative care such as ultrasound and strappings may be provided.
